IVD manufacturers with drug manufacturing licence can fill the membership form given below. Others (instrument manufacturers, suppliers and consultants) can apply for affiliated membership .

If your firm is a MSME, then you have to pay Rs.30,000.00 as one time membership fee and Rs.50,000.00 towards annual membership fee. If the firm is not an MSME, then you have to pay Rs.50,000.00 as one time membership fee instead of Rs.30,000.00 in the name of
'ASSOCIATION OF DIAGNOSTICS MANUFACTURERS OF INDIA'.

Also we required the following documents for records along with the form.

(1) MSME Udyog Aadhar(If Applicable).
(2) FDA License
